At Converge, we believe transparency is essential for a trusting therapeutic partnership; because of this core value, we ensure you will have access to your own health information, and ensure your voice is heard in your treatment process.
We want you to have all you need to request your records, understand your rights, and make use of available resources. You’ll also find ways to share feedback about your experience. In accordance with House Bill 4224 and Texas Health and Safety Code Section 181.105, Converge Couple Therapy PLLC provides the following:
Your health record belongs to you. Current and former clients may request copies of their counseling and mental health records maintained by Converge Couple Therapy PLLC.

To help prevent delays, please include the following information with your request:
- Client's full legal name and date of birth
• Current phone number and or email address
• Description of records requested, including specific dates if applicable
• Client signature and date
Identity verification may be required to protect client privacy and confidentiality in accordance with applicable state and federal laws. If records are requested on behalf of another individual, documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.
Delivery: To protect your privacy and maintain HIPAA compliance, we will deliver your records electronically through your secure client portal.
For Couple and Family Therapy clients:
Because we value the privacy of every individual in the family system, requests for records involving more than one person (such as couple or family therapy) require the written consent of all parties involved.
Administrative Costs: To cover the time and resources required to compile and securely transmit your files, a standard administrative fee of $25 to $50 is typically requested for full record sets. We provide these records within 15 business days of your written request.

Filing a Consumer Complaint:
